In order to participate in the event, you must submit the following medical forms and authorizations.
🛑 1. Required Forms
(Mandatory for all distances: 10K, 15K, 21K, 30K & 42K)
👨⚕️ Medical Certificate Form
- Requirement: It must be signed and stamped by a general practitioner or specialist, explicitly detailing and noticing your registered distance. We don’t require any specific exams, only medical confirmation that you are in optimal physical condition to complete the event.
- Valid formats and exceptions::
- If your country issues an annual sports certificate, we will accept it.
- If your doctor does not use a stamp, the form will only be accepted with their signature.
- You are permitted to submit a certificate from a similar event or one issued previously, provided it was signed/stamped within 6 months of the race (not before March 2026).
- It must be uploaded through our Google form in order to receive an automatic confirmation of its reception. 👉 Upload file:
- Importante: If you don’t have a Google account or the form fails, you can send it to: info@patagonianinternationalmarathon.com.
- ❌ Files sent via WhatsApp will not be accepted.
✍️ Liability Waiver & Release Form
- What does it cover?: It includes acceptance of risk, release of all liability, approval of the event’s rules & regulations, and authorization for media coverage.
- How to sign it?: On the date we indicated in the email, you will receive a direct email from Adobe Acrobat Sign in order to digitally sign it. Once signed, an automatic copy will be sent to both of us (nothing else needs to be done).
- Note: The PDF can be found in the documents folder, but we encourage you to only access it if you can’t digitally sign it as to avoid printing yet another form.

💊 Medical Record Form
- Required: It is mandatory to complete this form prior to the event and can be completed through the following link. Please include all health information you consider important/relevant in cases of emergency. Upon submission, you will receive an automatic confirmation of its completion.

♦️ 2. Documentation for Special Cases
(Only if applicable)
If the following situations do not apply to you specifically, please ignore this section:
- Runners under 18 years of age (Authorization for Minor): It must be signed by a parent or guardian. The PDF can be found in the folder, but we’ll send you an Adobe Acrobat Sign email so you can sign it digitally.
- Kit Retrievval by a Third Party (Race Kit Pick Up Authorization): This form allows a third party to collect your kit in Puerto Natales. It does NOT need to be sent before the event; however, a must be signed and presented by the person when retrieving the kit, including a copy of your ID (either a printed or digital copy).
